Elementary School Absentee Form

Students who are absent from school have three (3) school days to clear their absence. This form must be completed by the parent/guardian of the absent student. Please note submitting this form does not guarantee the absence will be excused.

Please allow 2-3 days for the absence to be excused. In addition, Scholars who are absent for 4 or more consecutive days must provide a doctor's note to excuse the absence. For more information on policies pertaining to absences, tardies and more please visit the Student Handbook.
