Middle School Absentee Form

Scholars who are absent from school have three (3) school days to clear their absence. This form must be completed by the parent/guardian of the absent Scholar, NOT by the Scholar. Please note that submitting this form does not guarantee the absence will be excused.

Please allow 2-3 days for the absence to be excused. Scholars who are absent for 3 or more consecutive days must provide a doctor's note to excuse the absence. In addition, if a Scholar is absent or has an early dismissal due to a doctor's appointment, a doctor's note must be provided to excuse the absence.
